چکیده
A double-blind study of the effects of isosorbide dinitrate, 10 mg given orally four times a day, propranolol, 40 mg four times a day, and the combination of these two drugs was performed on 21 patients with angina pectoris. Each patient received placebo, isosorbide, propranolol, and the combination of the two drugs for 1 month each in a random sequence over 4 months. The number of anginal pains and nitroglycerin tablets used were recorded, and a multistage treadmill ECG exercise test was performed after each treatment period. Frequency of anginal pains was reduced significantly with propranolol (7.0 2.5 pains/week) and the combination of propranolol and isosorbide dinitrate (9.9 + 2.9) as compared with placebo (21.0 + 6.4). Similarly, the number of nitroglycerin tablets was reduced with propranolol-containing regimens. Isosorbide was not significantly better than the placebo. Symptomatic improvement with propranolol could be related to a reduction in heart rate, and the product of heart rate and systolic blood pressure during exercise (P < 0.001). The capacity of these patients to perform a multistage exercise test, however, was not improved significantly, and the ischemic ST-segment changes

Propranolol in Angina Pectoris
The patients suffered no adverse effects when they were switched between treatment schedules. The average number of episodes of angina during the 4 weeks on long-acting propranolol was 7.3 and on standard propranolol, 6.3. Average nitroglycerin consumption was 5.8 and 4.9 tablets during therapy with these two drug programs. The resting values for heart rate, systolic blood pressure and rate-pre...
متن کاملTHERAPY AND PREVENTION ANGINA Superiority of combined diltiazem and propranolol therapy for angina pectoris
Twenty-four patients with stable angina were evaluated in a 14 week crossover trial. A single-blind placebo period (baseline 1) was followed by two double-blind periods evaluating maximum tolerated doses of diltiazem (up to 360 mg daily) vs placebo. Over the next 1 to 4 weeks, propranolol was started and increased until clinically documented f-blockade was achieved (baseline 2). The final phase...

Comparison of antianginal efficacy of one conventional and three long acting beta-adrenoreceptor blocking agents in stable angina pectoris.
We compared the antianginal efficacy of one conventional and three long acting beta-adrenoreceptor blocking agents in a randomised manner in 12 patients with stable angina pectoris. An exercise test was performed initially and in the 24th hour after a single daily dose of 160 mg of each beta-blocker at the end of a two week treatment period. In addition, glyceryl trinitrate consumption, anginal...
متن کاملGlyceryl trinitrate (nitroglycerin) ointment and isosorbide dinitrate: a review of their pharmacological properties and therapeutic use.
Sublingual glyceryl trinitrate (nitroglycerin) is the most widely used drug in the treatment of angina pectoris, but its use is limited due to its short acting effect. Recent investigations have shown that some longer acting nitrates administered orally or topically have a long acting antianginal effect. The mechanism of the antianginal effect of nitrates is multifactorial. Nitrates increase ox...
